4. Board of Accountancy - https://www.oregon.gov/BOA
5. Workers’ Compensation Board - https://www.oregon.gov/wcb
6. State Interoperability Executive Council- https://www.oregon.gov/siec

SharePoint Template Version 4.15.17 update that includes -
1. Update to Form Builder to support -
a. multi‑page forms and forms that display multiple fields on a single page.
b. file uploads for .txt, Word, and PDF documents up to 20 MB.

Anticipated down time: No downtime is anticipated.

Systems/Environments affected: SharePoint websites listed are expected to be impacted by this release. No additional systems or environments are expected to be impacted. Due to the architecture of the Microsoft SharePoint environment, if unexpected issues arise the SharePoint environment could be impacted, and a roll back will occur to restore the environment.
